• A negotiated fee structure with a network of experienced, capable dentists is available immediately to members. If your dentist is not currently a member of the provider list, you can help him or her investigate belonging to the network. If you want information about a dentist on their plan, they can provide that as well.
• Dentists’ credentials must be verified before they are listed on the plan.
• A monthly fee schedule is available for membership in the dental plan. Fees for individuals and families start at $7.00 a month. Group rates for 10 to 25 employees start at $5.00 per month. Rates for 26 or more employees begin at $4.00 a month. As a voluntary plan, there is no cost to the employer.
• The AVIA Dental Plan may be used as a supplemental plan for those who already have dental insurance.
• Acceptance as a member is automatic as soon as payment is made. Savings start immediately. There is no waiting to be approved as a member.
