Dominion 5000X Program Plan Features

This plan is offered by Dominion Dental Services to individuals who are not able to get dental coverage through their employers. The plan is also excellent for those individuals who are self-employed or who may have dental insurance but need supplemental coverage.

The following features are included in this plan.

• One annual membership fee covers either individuals or families.
• $10 a month covers an entire family.
• Free annual fluoride treatments for children are covered in the plan.
• Regular exams and an annual cleaning are free.
• Members receive reduced rates for 180 different dental services.
• If a dental service is required that is not on the list, the dentist will offer a 25 percent reduction from his usual fee for that service.
• The average fee reduction is 40 percent, with a range between 25 and 60 percent.
• Dependent coverage includes your spouse and all dependent children.
• Braces (orthodontia) are included.
