Plan Information
With the Patriot Dental Plan, you must visit a participating dentist to receive the discounted rates. All Patriot Plan dental providers are pre-screened and qualified using the high standards set forth by the National Committee for Quality Assurance.
Individual Membership: $99.95 per year
Family Membership: $149.95 per year
Dentists in the Patriot Plan network have contracted to provide discounted fees as outlined in each dentist’s discounted fee schedule. Any service or procedure that is not covered on the fee schedule will be discounted at 25%.
Membership
Once you enroll in the Patriot Dental Plan, you will receive a welcome package that includes a list of participating dentists, membership information, welcome letter, membership cards, fee schedule and instructions for using the plan. You will also be provided with support and contact information should you have any questions.
With the Patriot Dental Plan, you never have to bother with claim forms, meet deductibles or have limits imposed on dental services. You will have immediate coverage following your enrollment.
